I/we hereby authorize ABC Pediatrics, Prohealth Partners, a medical group, and Argus Medical Management, LLC to publish and use photograph(s)  that I/we have submitted by email or in person on this website.  I hereby release, discharge, and agree to save harmless ABC Pediatrics, Prohealth Partners, a medical group, and Argus Medical Management, LLC, its legal representatives or assigns, and all persons acting under their authority from any liability that may occur or be produced in the publishing of said  photograph(s), or in its subsequent publishing or processing thereof.

By submitting a photograph in person or by e mail, I/we have read and agree with the above.  I am aware that I/we can receive a copy of this consent if desired.
